HOSPITALTRADERS · PATIENT CONTINUITY SYSTEM™
A hospital shouldn't have to win the same patient again from zero, every time.
We help hospitals design, implement and monitor the journey after the first encounter — so appropriate patients stay connected to appropriate care.
THE PROBLEM
Busy hospital. Thousands of patients. But how many are actually connected to their next step?
You may recognize some of these situations.
Patients don't return for planned follow-up
Follow-up depends entirely on individual staff members
No structured recall system exists
Doctors lack a clear list of patients due for follow-up
Chronic patients disappear between visits
Preventive opportunities are missed
Patients move between departments without coordinated journeys
Discharge happens — but continuity doesn't
The hospital can't say how many patients were lost after the first visit
Marketing keeps acquiring new patients while existing relationships go undeveloped
The result: the hospital keeps working harder to acquire patients instead of systematically improving continuity of appropriate care.

THE LEAD DIAGNOSTIC
How strong is your Patient Continuity System?
We assess ten dimensions and return a Patient Continuity Score™ out of 100 — with a Red-Flag Rule so a serious gap can never hide behind a high overall score.
| Dimension | What we assess |
|---|---|
| Patient journey | Is the journey clearly mapped? |
| Follow-up | Are follow-ups structured? |
| Recall | Are patients systematically recalled? |
| Communication | Are reminders and education standardized? |
| Care coordination | Do departments coordinate? |
| Data | Can the hospital identify patients due for action? |
| Chronic care | Are chronic pathways structured? |
| Patient experience | Is continuity friction measured? |
| Staff ownership | Does every stage have an owner? |
| Dashboard | Can management monitor continuity? |

FAQ
Common questions
Is this a patient acquisition service?
No. Patient acquisition can be one component of the overall hospital strategy, but Patient Continuity System™ focuses primarily on what happens after patients enter the hospital's care journey.
Is this CRM software?
Not simply. The service combines patient journey design, workflows, people, communication, technology and management intelligence.
Do we need new software?
Not necessarily. Existing HIS, CRM, WhatsApp or other systems may be used where appropriate. Technology requirements depend on your current infrastructure.
Does this replace our hospital staff?
No. The model is designed to work with your existing team, with defined responsibilities, workflows and management oversight.
Can you work with our doctors?
Yes. Patient pathways are developed in alignment with the hospital's clinical leadership and approved protocols.
Can you guarantee more revenue?
No. The system is designed to improve visibility, process consistency and appropriate continuity. Financial outcomes depend on multiple factors and should be measured, not guaranteed.
Can this work for a 20–50 bed hospital, or a 200–300 bed hospital?
Yes to both. Scope and technology scale with hospital size and complexity; larger hospitals may need department-wise pathways and deeper dashboards.
