Documents to Apply For CGHS Empanelment
To apply for CGHS (Central Government Health Scheme) empanelment, you will need to submit certain documents. 3. Memorandum of Understanding (MoU): Prepare an MoU detailing the terms and conditions of empanelment, including the rates for different procedures, services, and facilities provided by your organization. This document should be signed and stamped by an authorized signatory.
4. Infrastructure details: Provide information about the infrastructure and facilities available at your healthcare facility. This may include details about the building, patient capacity, emergency services, diagnostic facilities, operation theaters, intensive care units, and other relevant amenities.
5. Certificates and registrations: Submit copies of the registration certificates of your organization, such as the Certificate of Incorporation, Memorandum of Association, and any other relevant certificates issued by the appropriate authorities.
6. Accreditation certificates: If your healthcare facility has any accreditation certificates from recognized organizations like NABH (National Accreditation Board for Hospitals and Healthcare Providers) or NABL (National Accreditation Board for Testing and Calibration Laboratories), include copies of those certificates.

7. List of doctors: Provide a list of doctors working at your healthcare facility along with their qualifications, experience, and specialization.

10. Infrastructure photographs: Include photographs of the infrastructure and key facilities at your healthcare facility, such as the reception area, patient rooms, operation theaters, diagnostic rooms, and other relevant areas.
11. Pan Card and GST registration: Attach copies of your organization's Pan Card and GST registration certificate.
12. Quality control policies: Provide details of your organization's quality control policies, including protocols for infection control, patient safety, and other relevant measures.
13. Any additional documents: If there are any additional documents specifically requested by the CGHS authorities, make sure to include them as well.
